It's about a girl who doesn't fit in to society's "perfect" girl. She puts on a fake smile and tries to be like them, but she keeps messing up, and they catch her at it. The people that care about her ask if she's okay, and she says she is, but she actually isn't. She's "Gonna hold her breath" and wait to be accepted. And she goes through a lot, and ends up wanting to kill herself, but than someone finds her and accepts her for who she is, and she gets better finally, and she can stop holding her breath.
This song shows that people aren't always happy like seem to say they are. You should accept them for who they are and don't make them follow societies "perfect" person. It can have dire consequences.
